ISO2026: Anabolic Osteoporosis Therapy in Cancer Patients

This session examines bone loss and fracture risk in patients receiving cancer therapies, with attention to hormone deprivation in breast cancer treated with aromatase inhibitors and in prostate cancer treated with androgen deprivation therapy. It reviews the magnitude of treatment-related bone loss, the scenarios of weight loss and low body mass index that compromise the skeleton, and the rationale for early densitometry. The session details the multiple roles of potent antiresorptive agents, including prevention of bone loss and fractures, adjuvant effects in postmenopausal breast cancer, and management of skeletal complications. It addresses the limited data on anabolic agents in oncology, the theoretical concerns regarding tumor activity within the bone microenvironment, and the circumstances in which anabolic therapy may be considered for patients who continue to fracture despite antiresorptive treatment.

Target Audience

This continuing education activity is intended for use by health professionals who participate in the care of patients at risk for or suffering from osteoporosis, in the areas of primary care, endocrinology, geriatrics, gynecology, internal, obstetrics, orthopedics, osteopathy, pediatrics, physiatry, radiology, rheumatology and physical therapy.

This includes physicians, nurse practitioners, registered nurses, pharmacists, physician assistants, technologists, researchers, public health professionals and health educators with an interest in osteoporosis and bone health.

Learning Objectives

Upon completion of this session, participants will be able to:
1.    Differentiate the mechanisms and magnitude of bone loss associated with aromatase inhibitor therapy in breast cancer and androgen deprivation therapy in prostate cancer.
2.    Describe the multiple indications for potent antiresorptive agents in oncology, including prevention of treatment-related bone loss, adjuvant benefit, and management of skeletal complications.
3.    Apply fracture-risk assessment and treatment-threshold principles to select antiresorptive therapy for patients receiving endocrine cancer treatment.
4.    Evaluate the available preclinical and clinical evidence on anabolic agents in patients with cancer, including theoretical risks related to the bone microenvironment.
5.    Identify the clinical circumstances in which anabolic osteoporosis therapy may be appropriate for cancer patients who fracture despite antiresorptive treatment.
